I had a cut wire and had to replace the IR Eye wire with a headphone cable. The IR puck has 5 connections and the headphone jack has three wires (green/ground, red/right, white/left). Does anyone know what wire goes to what three connections on the IR Eye? Thanks.

If you look at page 6.11 of the BS3 service manual you will see the pinout of the 9 pin IR connector.
If you just want to use IR control, then I think just 5V, IR and Ground need to be connected.

I think you can ignore Pins 1 and 2, as they relate to BtB (Hotel version control)
Pins 4 and 9 relate to light sense - but the BS3 manual states that they are not used (at least when the manual was produced).
Pins 3, 7 and 8 give IR control as already discussed.
This leaves pins 5 and 6, PT led and S.B led.
I would hazard a guess that these relate to 'Play Timer'(Green LED?) and 'Standby'(red LED?) respectively, hence connecting to the remaining two terminals on the puck would display these LED indications, if the IR eye has these. You could check whether the IR eye lights up in this manner by applying a small voltage (2-3V) to each terminal in turn (-ve to ground, obviously).
